Ozempic vs Mounjaro for type 2 diabetes
Ozempic (semaglutide) and Mounjaro (tirzepatide) are weekly injections for type 2 diabetes. In a head-to-head trial Mounjaro lowered blood sugar and weight more; Ozempic has the longer record on heart and kidney outcomes. Both are brand-only.

What the trials show
In SURPASS-2, people with type 2 diabetes on metformin who took tirzepatide had larger falls in A1c and body weight than those on semaglutide 1 mg. Semaglutide’s heart and kidney outcome trials led to label approvals that matter for people with established heart disease or kidney disease, and guidelines recommend a medicine with proven benefit in those situations. The best choice depends on which of those goals matters most for you.
Side effects
Both cause nausea, diarrhea, vomiting and constipation while the dose rises, and both carry the thyroid C-cell boxed warning and pancreatitis and gallbladder cautions. Both can cause low blood sugar when combined with insulin or sulfonylureas, whose doses are often reduced when either is started.
Cost
Neither has a generic, so ask your plan which it covers for type 2 diabetes and on which tier, since a plan can prefer one brand over the other. The manufacturers’ savings cards can lower the copay for eligible commercially insured patients, subject to each card’s terms. Medicare-negotiated prices for semaglutide take effect in January 2027.
Switching between them
People move between the two for side effects, coverage or response. Because the doses are not equivalent, the prescriber picks a starting dose on the new drug rather than converting directly, and often starts a step lower than the previous level to limit nausea.
