Bempedoic Acid
Bempedoic acid (Nexletol) is a once-daily tablet that lowers LDL cholesterol and, in adults at increased risk who cannot take recommended statin therapy, reduces the risk of major cardiovascular events; Nexlizet combines it with ezetimibe. Both are brand-only, and it can raise uric acid and bring on gout.
This page is general information about a medication and how cash prices for it are set. It is not medical advice and does not replace the label, your prescriber or your pharmacist. How we handle medical information
The FDA has not approved a generic of Nexletol or Nexlizet, so both are sold only as brands by one maker; ezetimibe on its own is sold as a generic by many makers.
About Bempedoic Acid
Bempedoic acid blocks ATP citrate lyase, an enzyme upstream of the one statins block, so the liver makes less cholesterol and clears more LDL cholesterol from the blood. The label, whose indications were revised in November 2025, covers two uses in adults: reducing the risk of major cardiovascular events (cardiovascular death, heart attack, stroke or coronary revascularization) in people at increased risk who are unable to take recommended statin therapy, including those taking no statin; and, with diet and exercise, lowering LDL cholesterol in hypercholesterolemia, including the inherited heterozygous form, alongside other cholesterol medicines or alone when those cannot be used. The dose is one 180 mg tablet a day, with or without food, and cholesterol is rechecked 8 to 12 weeks after starting.
There is no boxed warning, and the only contraindication is a past serious allergic reaction. It raises uric acid, usually within the first four weeks, which can bring on gout, so uric acid may be checked and gout symptoms, such as severe pain in a toe joint or a warm, swollen, red joint, should be reported. Tendon rupture is more likely over age 60, with corticosteroids or fluoroquinolone antibiotics, with kidney failure and after earlier tendon problems; the drug is stopped right away if a tendon ruptures, which can feel like a snap or pop. Upper respiratory infections, muscle spasms, back pain and abdominal pain are common; in the heart outcomes trial, kidney impairment, anemia, raised liver enzymes, gout and gallstones were also reported more often than with placebo.
Nexlizet adds 10 mg of ezetimibe, which blocks cholesterol absorption in the small intestine, to 180 mg of bempedoic acid in one tablet swallowed whole once a day; Nexlizet itself is approved to lower LDL cholesterol, and its bempedoic acid component carries the cardiovascular indication. It is taken at least 2 hours before or 4 hours after a bile acid sequestrant such as cholestyramine. With either product, simvastatin above 20 mg and pravastatin above 40 mg are avoided because of the risk of muscle problems, and triglycerides and HDL cholesterol are checked about 4 weeks after a fibrate is added. Based on how it works it may harm a fetus, so it is stopped when pregnancy is recognized unless the benefits outweigh the risks.
Taking Bempedoic Acid
- How to take it
- One tablet once a day, with or without food, exactly as prescribed; swallow Nexlizet whole, without cutting, chewing or crushing it.
- If you miss a dose
- Take it as soon as you remember; if it is almost time for the next dose, skip the missed one. Do not take two doses at once.
- Storage
- In the original package at room temperature, 68°F to 77°F (20°C to 25°C), out of reach of children; do not throw away the drying packet in the bottle.
- Interactions to know about
- Simvastatin above 20 mg and pravastatin above 40 mg are avoided. Take Nexlizet at least 2 hours before or 4 hours after cholestyramine or another bile acid sequestrant; cyclosporine levels are monitored, and fibrates other than fenofibrate are not recommended with Nexlizet. Corticosteroids and fluoroquinolone antibiotics add to the tendon risk. Tell the prescriber about gout, tendon problems, or severe kidney or liver problems.
This summarizes the official prescribing information. Your prescriber’s and pharmacist’s instructions, and the leaflet that comes with your medicine, take priority.
Why the cash price for Bempedoic Acid differs from pharmacy to pharmacy
Both products come from a single maker with no generic, so every quote is a brand price, and the gap between pharmacies is their purchase cost and markup. They come in bottles of 30, and a pharmacy that rarely dispenses them may need to order.
Generic ezetimibe costs pharmacies little, so Nexletol plus a separate ezetimibe tablet is priced differently from one Nexlizet tablet, with two dispensing fees instead of one; which comes out lower depends on the pharmacy, and whether two tablets suit you is the prescriber’s decision.

Ways to pay less for Bempedoic Acid without insurance
- Ask the maker whether its savings or patient assistance program applies to someone paying cash; many savings offers require commercial insurance.
- For Nexlizet, ask the pharmacy to quote it against Nexletol plus generic ezetimibe, and ask the prescriber whether two tablets would suit you.
- Ask for a ninety-day supply once you are settled on it.
- If it is quoted high, ask the prescriber whether generic ezetimibe alone, or a different statin or statin dose, would suit you.
- Upload the prescription to Script Unlock so licensed pharmacies can quote their cash price. You pay the pharmacy directly.
