    About Lumateperone

Lumateperone is an atypical antipsychotic; the label says its exact mechanism is unknown, but its effect is thought to come from blocking serotonin 5-HT2A receptors and acting on dopamine D2 receptors. It is approved for adults only: for schizophrenia; for depressive episodes of bipolar I or II disorder, alone or with lithium or valproate; and, since November 2025, as an add-on to an antidepressant for major depressive disorder. Safety and effectiveness in children have not been established. The usual dose for every use is 42 mg once a day, with or without food, and it does not need to be built up.

The boxed warnings say older people with dementia-related psychosis treated with antipsychotics have a higher risk of death, and lumateperone is not approved for them; and that antidepressants increased suicidal thoughts and behaviors in children and young adults, so patients are monitored closely for worsening and suicidal thinking. Other warnings cover stroke in older people with dementia, neuroleptic malignant syndrome, tardive dyskinesia, rises in blood sugar and cholesterol and weight gain, low white blood cell counts, dizziness or fainting on standing, seizures, and possible impairment of thinking and movement. Anyone allergic to lumateperone must not take it; itching, rash and hives have been reported. Sleepiness or sedation and dry mouth are the most common side effects, joined by dizziness and nausea in bipolar depression and by dizziness, nausea, tiredness and diarrhea in the depression add-on.

Lower 10.5 mg and 21 mg capsules, approved in 2022, are used when the dose must be reduced: 10.5 mg a day with a strong CYP3A4 inhibitor, and 21 mg with a moderate one or with moderate or severe liver impairment; CYP3A4 inducers are avoided. Babies exposed in the last three months of pregnancy may have movement or withdrawal symptoms after birth, and the National Pregnancy Registry for Atypical Antipsychotics follows outcomes. Lumateperone passes into breast milk in low amounts, with no data on effects on the infant, so the benefits of breastfeeding are weighed against the need for treatment.

    Taking Lumateperone

    How to take it
Once a day, with or without food, in the morning or at night, at around the same time each day; the full dose is taken from the start.
    If you miss a dose
Take it as soon as you can, but skip the missed dose if it is almost time for the next one. Do not take two doses at one time.
    Storage
At room temperature, 68°F to 77°F (20°C to 25°C), away from moisture and heat, out of reach of children.
    Interactions to know about
A strong CYP3A4 inhibitor such as itraconazole lowers the dose to 10.5 mg and a moderate one such as diltiazem to 21 mg; CYP3A4 inducers such as rifampin are avoided. Do not drive until you know how it affects you, and do not become too hot or dehydrated.

This summarizes the official prescribing information. Your prescriber’s and pharmacist’s instructions, and the leaflet that comes with your medicine, take priority.

Why the cash price for Lumateperone differs from pharmacy to pharmacy

Lumateperone has no generic, so every quote is for the same brand capsule and differs only by each pharmacy’s purchase cost, markup and dispensing fee. Most people take one 42 mg capsule a day; a pharmacy may keep only that strength on hand and need to order the 10.5 and 21 mg capsules.

Ways to pay less for Lumateperone without insurance

    • Ask the maker whether its savings or patient assistance program applies to someone paying cash; many copay cards require commercial insurance.
    • If you take a lower strength, confirm the pharmacy has it before the prescription is sent.
    • Once your dose is stable, ask for a ninety-day supply, and compare more than one pharmacy.
    • If cost is a barrier, ask the prescriber whether a generic medicine such as lurasidone or quetiapine would suit you.
    • Upload the prescription to Script Unlock so licensed pharmacies can quote their cash price. You pay the pharmacy directly.
