Riluzole
Riluzole (Rilutek, Tiglutik, Exservan) is a prescription medicine for amyotrophic lateral sclerosis (ALS), taken every 12 hours on an empty stomach as a tablet, a liquid suspension or a film that dissolves on the tongue. Generic 50 mg tablets come from more than one maker. It can injure the liver, so liver blood tests are done before and during treatment.
This page is general information about a medication and how cash prices for it are set. It is not medical advice and does not replace the label, your prescriber or your pharmacist. How we handle medical information
Generic riluzole 50 mg tablets are made by more than one company. A generic of the Tiglutik suspension has been approved but does not appear to be on the market yet, so check which form and which product the prescription names.
About Riluzole
Riluzole is approved for the treatment of ALS; the label states that how it works is unknown, and MedlinePlus notes that it may slow the disease but does not cure it. Every form is taken as 50 mg every 12 hours, at least one hour before or two hours after a meal, because food, especially a high-fat meal, reduces how much is absorbed.
The main warnings are liver injury, which has sometimes been fatal; a severe drop in the white cells that fight infection (neutropenia), seen within the first two months; and interstitial lung disease, including hypersensitivity pneumonitis, which means stopping the medicine at once. The suspension label, revised in July 2026, also warns of pancreatitis. Anyone who has had a severe allergic reaction to riluzole must not take it. The common side effects on the label are weakness, nausea, dizziness (more often in women), reduced lung function and abdominal pain.
Liver enzymes are measured before and during treatment, with checks for liver injury every month for the first three months and periodically after that; riluzole is not recommended when liver enzymes exceed five times the upper limit of normal. People with liver impairment and people of Japanese descent can have higher blood levels. The suspension can also be given through a feeding tube (PEG tube); the film dissolves on the tongue. Animal studies showed harm to the developing fetus, so discuss pregnancy plans with the prescriber.
Taking Riluzole
- How to take it
- Every 12 hours, at least one hour before or two hours after a meal. Shake the suspension for at least 30 seconds and measure it with the oral syringe supplied; place the film on top of the tongue with dry hands, take no liquid with it, and do not chew, spit or talk while it dissolves.
- If you miss a dose
- Take it as soon as you remember, unless it is almost time for the next dose; then skip the missed dose and carry on as usual. Do not take a double dose.
- Storage
- At room temperature, away from bright light, heat and moisture, out of reach of children. Keep the suspension upright, do not freeze it, and discard it 15 days after first opening; keep films in their sealed pouches.
- Interactions to know about
- CYP1A2 inhibitors such as ciprofloxacin, fluvoxamine and oral contraceptives can raise riluzole levels, and inducers can lower them; smoking speeds its clearance, and MedlinePlus advises avoiding charcoal-broiled foods. Other medicines that can harm the liver add to the risk. Report yellow eyes, dark urine, fever or a dry cough.
This summarizes the official prescribing information. Your prescriber’s and pharmacist’s instructions, and the leaflet that comes with your medicine, take priority.
Why the cash price for Riluzole differs from pharmacy to pharmacy
The tablet is an older generic, so quotes depend mostly on which generic a pharmacy stocks and how often it fills riluzole. A prescription that names Rilutek and blocks substitution is filled at a brand price.
The suspension and the film are priced as brand products and are not stocked everywhere. Suspension bottles are discarded 15 days after opening and films come in single-dose pouches by the carton, so the amount dispensed shapes the quote.

Ways to pay less for Riluzole without insurance
- If you can swallow tablets, ask the prescriber whether generic riluzole tablets would suit you, and ask the pharmacy for generic substitution.
- Once your dose and liver tests are stable, ask whether a ninety-day supply of tablets is possible.
- For the suspension, ask the pharmacy how many bottles it will dispense, since each opened bottle is discarded after 15 days.
- For Tiglutik or Exservan, ask the maker whether its savings or patient assistance program applies to someone paying cash.
- Upload the prescription to Script Unlock so licensed pharmacies can quote their cash price. You pay the pharmacy directly.
